Ontario Cup Entry Form

This online entry form is the registration process for the Ontario Cup.

The process is as follows:

1. Complete the online entry form. Please see below for a complete list of the information that you will need to complete this process
2. Pay the entry fee either online with your credit card or by cheque, money order or by e-transfer to etransferOS@ontariosoccer.net

Before your entry can be confirmed your application will need to be approved by your Club, your District Association and Ontario Soccer. You will be sent a system generated email to inform you when these approvals have occurred

To complete this entry form please ensure that you have the following information

1. The name and email address of the person in your Club who needs to approve your application
2. Your official Team Name in OSCAR
3. Your team's league and division
4. Your team's uniform and alternate uniform colours
5. The name, mailing address, phone numbers and email address of the person managing this team entry
6. The name, Ontario Soccer #, email address and phone numbers of your coaching staff and team managers
